Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC with strong clinical leadership in SNF settings.
• Collaborate with PCPs and therapy teams to manage common post-acute conditions.
• Track patient progress alongside therapy teams and remove barriers to care.
• Participate in multidisciplinary meetings and ongoing education.
