Comprehensive Rehab Consultants in Overland Park, KS is seeking a PM&R Clinician (NP/PA) to provide specialized PM&R services at a designated SNF or post-acute rehab center. The role offers autonomy, a flexible schedule around 9 AM–4 PM, and collaboration with an interprofessional team.

You will help manage conditions common in SNFs, work with PT/OT/SLP, and contribute to patient independence while benefiting from comprehensive benefits and ongoing education opportunities.

Qualifications

• Experience in post-acute space preferred.
• Ability to function autonomously in SNF/post-acute settings.
• Strong interpersonal skills and patient-centered approach.
• Empathy and compassion for geriatric populations.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCP teams for SNF care management.
• Coordinate with PT/OT/SLP teams to improve function.
• Track patient progress during therapy.
• Assess barriers to therapy and manage pain and neurogenic issues.
• Participate in multidisciplinary meetings and bridge primary care with internal medicine.
• Engage in ongoing education and regional team events.
