Comprehensive Rehab Consultants seeks a PM&R clinician (NP/PA) to provide specialized Physiatry services at a designated SNF/post-acute rehab center in Silvis, IL/East Moline, IL. The 32-hour, W-2 role offers autonomy, collaboration with therapy teams, and opportunities for clinical leadership.

You will work with PCPs, PT/OT/SLP teams, and facility staff to enhance patient outcomes, with a focus on geriatric post-acute care and continuous professional development.

Qualifications

• NP/APRN/PA license
• Illinois license
• Ability to work in the U.S.
• Experience in post-acute space preferred
• Great attitude and desire to deliver the best patient care

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant
• Represent CRC in your facility through clinical leadership and collaboration with the interdisciplinary care team
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA and postoperative orthopedic surgeries
• Work with PT, OT, and SLP to improve patient function and quality of life
• Follow therapy patients to track progress
• Assess and address barriers to therapy including pain, spasticity, neurogenic issues
• Attend multidisciplinary meetings and bridge therapy with internal medicine
• Engage in culture building and ongoing education including Didactic lectures, regional meetings, and dinners
