Job summary

Comprehensive Rehab Consultants (CRC) seeks a PM&R clinician to provide specialized Physiatry services at a designated SNF/post-acute rehab center in Aurora, CO. The role offers autonomy with a 32-hour W-2 schedule and a robust benefits package.

You will work with the post-acute therapy team, collaborating with PCPs and therapy staff to optimize patient outcomes and functional independence. CRC emphasizes leadership development and ongoing education.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA and postoperative surgeries.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and address barriers to therapy including pain, spasticity, neurogenic issues, etc.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
