Comprehensive Rehab Consultants is hiring a PM&R Clinician to provide specialized PM&R/Physiatry services at a designated SNF/post-acute rehab center in Coudersport, PA. The role emphasizes clinical leadership and collaboration with a multidisciplinary team to optimize patient independence.

The ideal candidate will be a licensed NP/APRN with PA licensure and experience in post-acute care, ready to work autonomously with flexible weekday hours and supported by CRC’s training and resources.

Qualifications

• NP/APRN license required.
• PA license required.
• Ability to work in the U.S.
• Post-acute/post-acute space experience preferred.
• Strong patient-care attitude and professional conduct.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage SNF/post-acute conditions.
• Work with PT, OT, and SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
