Job summary

Comprehensive Rehab Consultants (CRC) seeks an NP/PA PM&R clinician to provide specialized Physiatry services at a designated SNF/post-acute rehab center in Middleborough, MA. You will work with an interdisciplinary team to optimize rehabilitative outcomes and promote patient independence, with autonomy over daily workflow.

The ideal candidate has post-acute experience, strong leadership, empathy, and excellent collaboration skills.

Qualifications

• Nurse Practitioner/APRN/PA license.
• Massachusetts license or eligibility.
• Ability to work in the U.S. and post-acute space experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with facility staff and interdisciplinary care teams.
• Support management of SNF/post-acute conditions (CVA, neurological disorders, post-op ortho).
• Coordinate with PT/OT/SLP to improve function and independence.
• Attend multidisciplinary meetings and drive clinical leadership.
