Job summary

Comprehensive Rehab Consultants is seeking a part-time PM&R Clinician to serve a designated SNF/post-acute center in Coudersport, PA. You will deliver specialized PM&R care, collaborate with PCPs, and work closely with PT/OT/SLP teams to enhance patient independence and outcomes.

The role emphasizes autonomy without on-call requirements, with in-services and ongoing education to support clinical leadership. Requires NP/APRN and PA licenses with post-acute experience preferred.

Qualifications

• Nurse Practitioner/APRN license required.
• Pennsylvania license required to practice in US.
• Post-acute/SNF experience preferred.
• Ability to work autonomously and collaborate with teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with clinical leadership.
• Collaborate with PCPs for SNF conditions and surgeries.
• Coordinate with PT/OT/SLP to improve function.
• Track patient progress and follow therapy notes.
• Attend multidisciplinary meetings and support culture.
