Job summary

Comprehensive Rehab Consultants (CRC) in the United States seeks a PM&R clinician to provide physiatry services at designated SNFs. You will collaborate with post-acute therapy teams to optimize rehabilitation and patient independence, with daily workflow autonomy and no on-call obligation.

Ideal candidates have SNF or post-acute experience, strong leadership, and a compassionate approach to geriatric care, with opportunities for ongoing education and career growth.

Qualifications

• Nurse Practitioner, APRN, or PA license in Virginia.
• Post-acute/SNF experience preferred.
• Ability to work autonomously in a clinical setting.

Responsibilities

• Provide PM&R/physiatry services as a rehab consultant.
• Collaborate with the interdisciplinary care team.
• Support SNF patients with CVA and postoperative care.
• Coordinate with PT/OT/SLP to improve function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
