Comprehensive Rehab Consultants (CRC) seeks a PM&R clinician to provide specialized Physiatry services in post-acute SNF settings, collaborating with therapists and PCPs to optimize rehabilitation outcomes.

The ideal candidate will have NP/PA licensure in Texas, with autonomy to manage patient care and a commitment to geriatric care. The role offers a 32-hour W-2 schedule in Kountze/Silsbee, TX with a competitive salary and benefits.

Qualifications

• Interest in post-acute healthcare and geriatric populations.
• Ability to function autonomously within SNF/post-acute settings.
• Strong interpersonal skills to coordinate with facility partners and patients.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNFs.
• Lead clinically within the facility and collaborate with interdisciplinary teams.
• Assist PCPs in managing common SNF conditions (CVA, neurological conditions, postoperative ortho).
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow patients to track therapy progress and address barriers to care.
• Participate in multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture-building and ongoing education, including didactics and regional meetings.
