Job summary

Comprehensive Rehab Consultants (CRC) seeks a Part-time PM&R Clinician to provide specialized Physiatry services at a designated SNF in Greendale, WI. The role partners with rehab teams to optimize patient independence and outcomes.

Responsibilities include leading in interdisciplinary settings, collaborating with PCPs, and promoting continuous improvement in post-acute care. The package features a laptop, malpractice coverage, CEU opportunities, and a 401(k) plan.

Qualifications

• NP/APRN/PA license or eligibility.
• Wisconsin license required or in process.
• Ability to work in the United States.
• Post-acute/SNF experience preferred.
• Desire to deliver high-quality patient care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCP teams to manage SNF conditions.
• Coordinate with PT/OT/SLP to improve patient function.
• Track progress of therapy and follow patients through rehab.
• Identify and address barriers to therapy (pain, spasticity, neurogenic issues).
• Participate in multidisciplinary meetings and culture-building activities.
