Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with PCP teams and interdisciplinary care staff.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow therapy patients to track progress and outcomes.
• Bridge therapy and internal medicine within the SNF team.
