Job summary

Comprehensive Rehab Consultants is seeking a PM&R clinician (NP/PA/APRN) to provide specialized Physiatry services at a designated SNF in Greendale, WI. This part-time, 16-hour W2 role focuses on improving patient independence through close collaboration with therapy and medicine teams.

You will enjoy autonomy with flexible hours (around 9 AM–4 PM) and access to a robust training path, malpractice coverage, CEUs, and a supportive culture designed for growth and work-life balance.

Qualifications

• Licenses required to practice in Wisconsin as NP/PA/APRN.
• Experience in post-acute space preferred.
• Ability to work autonomously in SNF/post-acute settings.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with PCP and interdisciplinary care teams.
• Work with PT/OT/SLP to improve patient function.
• Attend multidisciplinary meetings and lead facility care.
• Support documentation and care coordination for SNF patients.
