Comprehensive Rehab Consultants in Greendale, WI, is seeking a part-time PM&R clinician (NP/PA) to provide specialized rehabilitation services at a designated SNF. You will collaborate with an interdisciplinary team to optimize patient outcomes and independence, with a focus on geriatric post-acute care.

Ideal candidates hold an NP/PA license, Wisconsin license, and have post-acute experience. The role offers weekday hours around 9 AM–4 PM, on-site with competitive benefits and malpractice

Qualifications

• NP/APRN/PA license required and Wisconsin license valid to practice.
• Post-acute space experience preferred.
• Ability to work in the U.S. and serve geriatric/post-acute patients.
• Strong interpersonal skills and empathy to support team and patients.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant at a SNF/post-acute center.
• Collaborate with interdisciplinary care teams and PCPs to manage conditions typical in SNFs.
• Work with therapy teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
