Comprehensive Rehab Consultants (CRC) seeks a Part-time PM&R Clinician to provide Physiatry services exclusively at a designated SNF/post-acute center in Bloomfield, NM. The role emphasizes autonomy, collaboration with therapy and medical teams, and ongoing education.

The position offers a 16-hour week with standard 8 AM–5 PM hours, a remote scribe support for documentation, and a benefits package including 401(k), malpractice insurance, CEUs, and access to specialty certification.

Qualifications

• Empathy and compassion for geriatric patients in SNF/post-acute settings.
• Strong interpersonal skills to collaborate with facility staff and patients.
• Ability to work autonomously and adapt to changing conditions in care delivery.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility with clinical leadership and teamwork.
• Collaborate with PCP teams to manage SNF-related conditions such as CVA and postoperative surgeries.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Participate in multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education including Didactic lectures and regional meetings.
