Job summary

Comprehensive Rehab Consultants is seeking a part-time NP/PA PM&R Clinician to provide Physiatry services at a designated SNF or post-acute rehab center in Bloomfield, NM.

You will collaborate with the post-acute therapy team to promote patient independence, with typical hours about 8 AM–5 PM and a 16-hour schedule. The role includes malpractice coverage, CEU opportunities, and 401(k) benefits to support ongoing clinical growth.

Qualifications

• NP/APRN/PA license and NM state license required.
• Post-acute/SNF experience preferred.
• Ability to work in the U.S.
• Passion for geriatric post-acute healthcare.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with strong clinical leadership.
• Collaborate with PCP teams for SNF conditions like CVA and neurological issues.
• Work with PT/OT/SLP teams to improve function and quality of life.
• Monitor patient progress and follow therapy outcomes.
• Assess barriers to therapy such as pain or neurogenic issues.
• Bridge therapy and internal medicine in multidisciplinary meetings.
• Engage in culture-building and ongoing education.
