Comprehensive Rehab Consultants is seeking a PM&R/Physiatry clinician to deliver specialized services in a designated SNF or post-acute rehab center in Bloomfield, NM.

The role emphasizes autonomous practice, collaboration with PCPs and therapy teams, and ongoing education to advance rehabilitation outcomes for geriatric patients. Hours are typically 8 AM–5 PM, with a flexible weekday schedule.

Qualifications

• Ability to work autonomously in SNF/post-acute settings.
• Strong interpersonal and communication skills for interdisciplinary teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute centers.
• Represent CRC with clinical leadership in the facility and collaborate with care teams.
• Collaborate with PCPs to manage common SNF conditions and postoperative needs.
• Work with PT/OT/SLP teams to improve patient function and QoL.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
