Job summary

Comprehensive Rehab Consultants is seeking a PM&R Clinician to deliver Physiatry services at a designated SNF/post-acute rehab center in Gallup, NM. This part-time W2 role offers daytime hours and close collaboration with therapy teams to optimize patient outcomes.

Qualifications

• NP/APRN/PA license required.
• New Mexico license required.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.
• Desire to deliver high-quality patient care.
• Strong interpersonal and communication skills.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
• Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess barriers to therapy including pain, spasticity, neurogenic bowel and bladder, and related issues.
• Become part of the SNF team by attending multidisciplinary meetings and bridging therapy with internal medicine.
• Engage in culture building and ongoing education including monthly didactic lectures and regional activities.
