Comprehensive Rehab Consultants is seeking a part-time PM&R clinician to provide Physiatry services at a designated SNF in Clovis, NM. The role emphasizes autonomy, collaboration with an interdisciplinary team, and improving geriatric patient function in a post-acute setting.

The position offers flexible hours around 9 AM–4 PM, with a focus on quality patient care and ongoing professional development. Requires NP/PA license or equivalent and NM licensure.

Qualifications

• Post-acute/SNF experience preferred.
• Ability to work autonomously.
• Strong patient-focused care.

Responsibilities

• Provide PM&R services as a rehab consultant.
• Represent CRC with facility through clinical leadership and collaboration with the care team.
• Collaborate with PCPs to manage SNF conditions such as CVA and neurological issues.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and address barriers to therapy.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
• Engage in culture-building and ongoing education.
