Qualifications

• Nurse Practitioner/PA license valid in Virginia.
• Post-acute/SNF experience preferred.
• Autonomy and collaboration with care teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facilities with clinical leadership.
• Collaborate with PCPs to manage SNF conditions.
• Work with PT/OT/SLP to improve function and QoL.
• Follow therapy patients to track progress.
• Attend multidisciplinary meetings and support culture.
