Comprehensive Rehab Consultants (CRC) is seeking a PM&R/Physiatry clinician to deliver specialized care exclusively at SNFs or post-acute rehab centers in Mountain View, CA. The role emphasizes autonomy, collaboration with therapy and primary care teams, and a path to clinical leadership.

Qualifications

• NP/APRN/PA license required.
• California license required.
• Ability to work in the U.S.
• Post-acute space experience preferred.
• SNF or post-acute care experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and interdisciplinary collaboration.
• Collaborate with PCP teams for management of SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Bridge therapy and internal medicine by attending multidisciplinary meetings.
• Engage in culture-building and ongoing education including monthly didactics and regional events.
