Job summary

Comprehensive Rehab Consultant seeks a part-time PM&R clinician to provide specialized Physiatry services within SNF settings in Modesto, CA. You will collaborate with PCPs and rehab teams to optimize patient outcomes and participate in ongoing education.

You must hold NP/PA licensure in California, be able to work in the U.S., and embrace autonomous, patient-centered care with strong leadership in a growing CRC team.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• CA license required.
• Eligibility to work in the U.S.
• Post-acute care experience preferred.
• Autonomy and collaboration with physicians and care teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facilities with strong clinical leadership.
• Collaborate with PCPs to manage post-acute conditions.
• Work with PT, OT, and SLP teams to improve patient function.
• Follow therapy patients to track progress.
• Assess and manage barriers to therapy (pain, spasticity, etc.).
• Participate in multidisciplinary meetings and promote culture-building.
• Engage in ongoing education and regional activities.
