Comprehensive Rehab Consultants (CRC) is seeking a Nurse Practitioner/PA to provide PM&R services at designated SNF or post-acute rehab centers in Modesto, CA. This W-2, full-time role emphasizes autonomy, collaboration with interdisciplinary teams, and improving patient independence.

The ideal clinician has post-acute or geriatric care experience, a California license, and a patient-centered approach. CRC offers training, strong benefits, and opportunities for career growth.

Qualifications

• Post-acute healthcare experience is preferred.
• California license to practice in the U.S.
• Ability to work in the U.S. required.
• Great attitude and patient-centered care.
• 40 hours/week with flexible scheduling.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCP teams for SNF/post-acute conditions.
• Coordinate with PT/OT/SLP to improve patient function.
• Track patient progress in therapy and outcomes.
• Address barriers to therapy and optimize care.
• Participate in multidisciplinary meetings and culture-building.
