Comprehensive Rehab Consultants (CRC) is seeking a PM&R Clinician (NP/PA) to serve in a designated SNF/post-acute rehab setting in Mountain View, CA. The role emphasizes autonomous clinical leadership, collaboration with PT/OT/SLP teams, and bridging therapy with internal medicine to optimize patient function and outcomes.

The ideal candidate has post-acute care experience, a California license, and a passion for geriatric rehabilitation.

Qualifications

• NP/APRN/PA license required.
• California license required.
• Experience in post-acute care preferred.
• Desire to deliver patient-centered care and contribute to team growth.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute centers.
• Represent CRC with strong clinical leadership within the facility.
• Collaborate with PCPs and therapy teams to manage common SNF conditions.
• Assist in bridging therapy and internal medicine for comprehensive care.
• Attend multidisciplinary meetings and contribute to culture-building and education.
