Job summary

Comprehensive Rehab Consultants (CRC) in Gallup, NM is seeking a Part‑time PM&R clinician (NP/PA) to provide rehabilitative services at a designated SNF. You will work with the post‑acute therapy team to optimize patient outcomes and independence.

This physician‑owned group offers autonomy, growth opportunities, a supportive culture, malpractice coverage, and a daytime schedule focused around 9 AM–4 PM.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• New Mexico license required.
• Ability to work in the U.S. with post-acute space experience preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage conditions common in SNFs (CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and collaborate on barriers to therapy (pain, spasticity, neurogenic issues).
• Be integral to SNF team by attending multidisciplinary meetings and bridging therapy with internal medicine.
• Engage in culture-building and ongoing education including Didactic lectures and regional meetings.
