Job summary

Comprehensive Rehab Consultants in Shrewsbury, MA is seeking a PM&R clinician (NP/PA) to deliver specialized Physiatry services at a designated SNF or post-acute center. The role is a 32-hour, W-2 position with clinical autonomy within a growing, physician-owned group.

You will collaborate with the rehab team to optimize outcomes, lead with the interdisciplinary care plan, and support patients toward independence, while benefiting from competitive pay, full-time support, and ongoing education

Qualifications

• Post-acute care experience preferred.
• Active NP/PA/ APRN license in MA required.
• Strong clinical leadership and teamwork abilities.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with the interdisciplinary team.
• Collaborate with PCPs on SNF/post-acute conditions.
• Work with PT/OT/SLP to improve function and quality of life.
• Track patient progress and follow therapy outcomes.
• Address barriers to therapy and coordinate care.
• Participate in multidisciplinary meetings and education sessions.
