Comprehensive Rehab Consultants is seeking a PM&R clinician to provide rehabilitative services at designated SNFs and post-acute centers. This full-time role offers autonomy in daily workflow, collaboration with an interdisciplinary team, and a focus on optimal patient independence.

The ideal candidate will hold NP/PA licensure valid in the U.S., with Maine licensure preferred, and a passion for geriatric post-acute care. Competitive compensation and growth opportunities are provided.

Mansioni

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage SNF conditions (CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with your PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education including monthly Didactic lectures and regional meetings

Conoscenze

Nurse Practitioner/APRN/PA license
