Job summary

Comprehensive Rehab Consultant in East Longmeadow, MA is hiring a PM&R clinician (NP/PA) to provide specialized Physiatry services in SNF settings. This physician-owned group emphasizes autonomy, excellent patient care, and work-life balance with no nights or on-call requirements.

You will collaborate with PCPs and therapy teams, manage post-acute conditions, participate in multidisciplinary meetings, and access extensive training, CEUs, and education reimbursement.

Qualifications

• A passion for post-acute healthcare and geriatric care.
• Strong autonomy and collaboration with interdisciplinary teams.
• Comfort working in SNF/post-acute settings.
• Excellent interpersonal and communication skills.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNFs.
• Represent CRC with clinical leadership in facilities and collaborate with care teams.
• Coordinate with PCPs to manage post-acute conditions (CVA, neurological, postoperative ortho).
• Work with PT/OT/SLP teams to improve function and quality of life.
• Attend multidisciplinary meetings and contribute to culture and education.
