Comprehensive Rehab Consultants in Lanham, MD is hiring a PM&R Clinician (NP/PA) to deliver rehabilitation services at designated SNFs. You will collaborate with the post-acute therapy team to optimize patient outcomes and promote independence, treating diverse geriatric populations in a structured, outpatient-like setting.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with facility staff and interdisciplinary care team.
• Co-manage SNF conditions with PCPs (CVA, neurological, postoperative ortho).
• Work with PT/OT/SLP to improve function and QoL.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Support culture-building and ongoing education within the team.
