Job summary

Comprehensive Rehab Consultants in Shrewsbury, MA seeks a NP/PA PM&R Clinician to provide specialized PM&R services at a designated SNF/post-acute rehab center. You will collaborate with PCPs and therapy teams to optimize functional outcomes.

Qualifications

• Post-acute care experience preferred.
• Ability to function autonomously.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with facility team and PCPs to manage SNF/post-acute conditions.
• Work with therapy teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
