Job summary

Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA) to provide Physiatry services at designated SNFs. You will lead clinical care with an interdisciplinary team in a post-acute setting, contributing to improved patient independence.

Ideal candidates are licensed NPs/PAs with post-acute experience who can work autonomously and participate in ongoing CRC education and leadership opportunities.

Qualifications

• Nurse Practitioner/APRN/PA License.
• Connecticut and/or Rhode Island License.
• Ability to work in the U.S.
• Experience in post-acute space preferred.
• Great attitude and desire to deliver the best patient care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage common SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy including pain, spasticity, neurogenic issues.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
• Engage in culture building and ongoing education including Didactic lectures and regional events.
