Comprehensive Rehab Consultants (CRC) seeks an NP/PA/Physician Assistant to deliver PM&R services at a designated SNF or post-acute rehab center in Shrewsbury, MA. The role emphasizes autonomy, leadership, and collaboration with an interdisciplinary team to promote patient independence.

You will contribute to care plans, work closely with PCPs and therapy colleagues, and participate in ongoing education and certification initiatives, including CRC’s own training and CEU opportunities.

Qualifications

• Licensed NP/PA/APRn with ability to practice in Massachusetts.
• Experience in post-acute or SNF settings preferred.
• Strong collaboration with PCPs and therapy teams required.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Lead and collaborate with the interdisciplinary care team in the SNF.
• Support management of common SNF conditions and postoperative care.
• Coordinate with PT/OT/SLP to optimize patient function and independence.
• Attend multidisciplinary meetings and contribute to culture building.
