Job summary

Comprehensive Rehab Consultants in Hot Springs, VA, seeks a PM&R Clinician (NP/PA) to provide Physiatry services in a designated SNF, working part-time on a 16-hour W2 basis. You will collaborate with the post-acute therapy team to improve patient function and independence.

The ideal candidate holds an NP/APRN/PA license in Virginia, has post-acute or SNF experience, and can work weekdays 8 AM–5 PM with autonomy and opportunities for ongoing education including CEUs.

Qualifications

• Active Virginia NP/PA/APRN license required.
• Experience in post-acute/SNF settings preferred.
• Ability to work autonomously with strong clinical leadership.

Responsibilities

• Provide PM&R/Physiatry services as rehab consultant in SNF.
• Represent CRC in facility through collaboration with the interdisciplinary care team.
• Coordinate with PCP teams to manage stroke and other post-acute conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Track patient progress and address barriers to therapy.
