Qualifications

• Post-acute care experience preferred.
• Ability to work autonomously within SNF settings.
• Strong collaboration with interdisciplinary teams (PT/OT/SLP).
• Empathy and compassion in geriatric care.
• Willingness to lead and mentor facility staff.

Responsibilities

• Provide PM&R/Physiatry services at SNF or post-acute center.
• Represent CRC with strong clinical leadership in the facility.
• Collaborate with PCP teams on common SNF conditions.
• Coordinate with PT/OT/SLP to improve patient function.
• Follow patients to track progress and barriers to therapy.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in ongoing education and regional didactics.
