Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA/APR N) to deliver Physiatry services at a designated SNF/post-acute center in Connecticut. You will work closely with an interdisciplinary team, drive patient independence, and participate in ongoing education and leadership activities.
The role offers standard 8-hour days, a 3-day work week option, and a competitive salary with bonus potential. Strong commitment to post-acute geriatric care and autonomy is essential.
Qualifications
- Post-acute healthcare experience preferred.
- Ability to work autonomously within SNF/post-acute settings.
- Strong collaboration with interdisciplinary teams.
Responsabilités
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility and lead clinical teams.
- Collaborate with PCPs to manage SNF conditions.
- Coordinate with PT/OT/SLP to improve patient function.
- Track progress of therapy patients and adjust plans.
- Identify and address barriers to therapy (pain, spasticity, neurogenic issues).
- Participate in multidisciplinary meetings and contribute to culture building.
- Support ongoing education and regional team communications.
