Job summary

Comprehensive Rehab Consultants is seeking a PM&R clinician to deliver specialized Physiatry services at designated SNFs or post-acute centers in Connecticut. The role emphasizes collaboration with therapy and medical teams to optimize patient independence and functional outcomes.

You will join a physician-owned group that values accountability, patient-centered care, and ongoing professional development, including opportunities for CMS and facility-level leadership within post-acute care

Qualifications

• Nurse Practitioner/APRN/PA license required; CT license preferred.
• Experience in post-acute SNF care is preferred.
• Strong interpersonal skills, empathy, and autonomous work style.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Lead and collaborate with interdisciplinary care team.
• Support SNF patients with mobility and functional goals.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Track patient progress with PT/OT/SLP colleagues.
