Northeast Healthcare Recruitment is seeking an experienced physician to join our clinical team in a Part Time capacity, at least 3 days per week, to provide primary and episodic care for residents in skilled nursing facilities.

This independent role emphasizes continuity of care; you’ll manage your own panel, chart from home, and build lasting relationships with patients and their families, with a 6-hour shift pattern and a strong emphasis on autonomy and flexibility.

Qualificações

• MD or DO from an accredited medical school.
• Board Certified (BC) in Internal Medicine, Family Medicine, or Geriatrics (or equivalent).
• Valid, unrestricted physician license in the state of operation.
• Must be opted-in with Medicare.
• Successful completion of an accredited residency program.
• Skilled nursing or long-term care experience strongly preferred.

Responsabilidades

• See 16–25 patients per day in a mix of long-term chronic care and acute issues.
• Perform annual wellness visits and H&Ps; manage chronic and co-morbid conditions.
• Coordinate with specialists and facility staff to ensure seamless care; participate in care conferences when appropriate.

About the Role

Your Day-to-Day

You'll see roughly 16–25 patients a day a mix of long-termchronic care and acute issues. That means annual wellness visits and H&Ps,managing chronic and co-morbid conditions, responding to acute changes,reviewing meds and orders, and coordinating with collaborating specialists andfacility staff to keep care seamless. You'll also stay in touch with familiesand join care conferences where it makes sense.
