Job summary

Evolve Therapy Services in Oklahoma City, OK is seeking a Physical Therapy Assistant to support an in-house skilled nursing facility. You will work under the supervision of a Physical Therapist to provide resident-centered care and contribute to treatment plans.

The position offers varied shifts including full-time, part-time, or PRN. Candidates must hold an Associate degree in physical therapy and an active PTA license, with commitment to ongoing education and licensure maintenance.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluating resident progress and updating treatment plans with the PT.
• Participate in developing short- and long-term goals and treatment plans.
• Deliver individual and group therapy focusing on strength, mobility, balance, and ROM.
• Educate residents, families, and staff on therapy techniques and safety.
• Document compliant CMS-ready notes in NetHealth EMR as required.
• Coordinate schedules and maintain timely attendance for therapy sessions.
