Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Ponca City, OK. This PRN position supports the Physical Therapist in delivering patient-centered therapy services.

Responsibilities include screenings, evaluations under supervision, developing treatment plans, and implementing therapy activities to improve residents' mobility and function, while adhering to CMS guidelines and ensuring quality care.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under the Physical Therapist guidance.
• Implement treatment plans including strength, mobility, balance, gait, and modalities.
• Monitor resident response and modify as directed by the Physical Therapist.
• Assist in scheduling and coordinating resident therapy sessions.
