Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Lawton, OK. This full-time or PRN position supports the therapy team under the supervision of a Physical Therapist.

The role involves conducting screenings, assisting in evaluations and treatment plans, and delivering both individual and group therapies focusing on mobility, strength, gait, balance, and ROM. CMS‑compliant documentation in NetHealth EMR is required.

Qualifications

• Active Physical Therapy Assistant license in state of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in developing treatment plans.
• Assist in establishing target goals and plans with the Physical Therapist.
• Conduct individual and group treatment focusing on strength, mobility, gait, balance, ROM and modalities.
• Select and adapt PT techniques/media in consultation with the PT.
• Monitor residents’ response and modify interventions as needed.
• Assist in scheduling and coordinating resident therapies with the team.
• Document compliant and necessary CMS-mandated records in NetHealth EMR.
