Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant to support an in-house Skilled Nursing Facility in Midwest City, OK. You will assist the supervising Physical Therapist in planning, implementing, and documenting resident PT services to ensure high-quality care.

The role requires an Associate's degree in physical therapy and an active PTA license, with opportunities for ongoing education and growth.

Qualifications

• Active Physical Therapy Assistant license required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluating residents under the direction of the Physical Therapist.
• Assist in developing target goals and treatment plans.
• Deliver individual and group treatments including strength, mobility, gait, ROM, and modalities.
• Monitor residents' response to intervention and modify as needed.
• Assist in scheduling in collaboration with the PT and care team.
• Document CMS-required patient care activities in NetHealth EMR.
