Job summary

Qualifications

• Associate degree in physical therapy from an accredited program.

Responsibilities

• Provide PT services under supervision of PT.
• Conduct resident screenings at regular intervals.
• Assist in evaluating and setting goals with the PT.
• Contribute to treatment plans and progress notes.
• Deliver individual and group therapy sessions as directed.
• Document compliant, medically necessary CMS documentation.
