Job summary

Carle Health seeks a Licensed Physical Therapist Assistant to evaluate and treat patients of all ages, adjusting plans as needed and serving as a clinical resource for staff. This role offers a sign-on bonus and relocation assistance for movers of 50+ miles.

Responsibilities include educating patients and families, following and adjusting the plan of care, and documenting per policy. Collaborative, patient-centered care is required at this Carle Health facility.

Qualifications

• Associate's Degree in Physical Therapy Assistant
• Licensed PTA (IDFPR) in Illinois or eligible to practice
• BLS certification (AHA) within 30 days
• Board eligible and completion of board exam when date is available

Responsibilities

• Educates patients, family, and caregivers using effective teaching methods.
• Follow and adjust plan of care based on patient response and evidence in daily notes.
• Document care per CTSCP67 and annual education for DNV Hip and Knee program (if applicable).
• Follow PT plan of care and coordinate with PT on patient treatment interventions.
• Provide therapeutic exercise, gait and balance training, ultrasound, electrical stimulation, and aquatic therapy.
• Complete daily documentation and charging in therapy EMR.
• Communicate progress to referring provider and document treatment recommendations.
