Caretenders Home Health, part of LHC Group within the Optum family, seeks a licensed Physical Therapist Assistant to support patient well‑being under a supervising PT. You will implement the plan of care and educate patients and families on exercises and safety at home.

Your role involves in‑home treatment, documentation, and collaboration with nurses and physicians to ensure quality care and discharge readiness. Weekend rotation and a valid state's PTA license are required.

Qualifikationen

• Current, licensed/registered/certified to assist in physical therapy in state of practice.
• Available to work a weekend rotation.
• CPR certification preferred or must be obtained within 90 days of hire.

Aufgaben

• Follow state laws and plan of care as defined by supervising PT.
• Instructs patients in active and passive exercises, gait training, ADL, transfers and safety procedures.
• Observe, document and report patient responses and coordinate with the health care team.
