Job summary

Caretenders Home Health, part of the LHC Group and affiliated with UnitedHealth Group, seeks a Physical Therapist Assistant to deliver PT services in home health care under the supervising PT’s plan of care.

Responsibilities include instruction in exercises, gait and mobility, patient education, and coordination with the care team. Candidates must be licensed PTA and have a valid driver’s license with access to reliable transportation; weekend shifts may be required.

Qualifications

• Licensed PTA in state of practice.
• Current driver’s license and access to a vehicle or reliable transportation.
• Weekend rotation availability.

Responsibilities

• Follow state-specific laws governing home PT and implement the plan of care.
• Instruct patients in active/passive exercise, gait, ADL, transfer, and safety.
• Observe, document and report patient progress to the supervising PT and care team.
• Educate patient, family and health team on therapy within PTA scope.
• Provide care in varied home settings with sensitivity to all individuals.
