Physical Therapist - Viverant Physical Therapy

Join to apply for the Physical Therapist - Viverant Physical Therapy role at Infinite Health Collaborative.

Description: The Physical Therapist is responsible for delivering exceptional patient care, performing evaluations, and developing treatment plans for outpatient conditions. This is a full-time position with flexible hours at Anytime Fitness in Maple Grove. Viverant Physical Therapy is part of Infinite Health Collaborative (i-Health), a group of local medical practices owned and led by physicians, aiming to give patients more control over their healthcare.

Essential Functions:
  1. Evaluate and treat outpatient conditions in consultation with physicians.
  2. Evaluate and treat outpatient conditions under direct access.
  3. Apply therapeutic exercises, modalities, manual therapy, and other treatments according to state practice laws.
  4. Document patient care services per regulatory and departmental guidelines.
  5. Supervise a Physical Therapist Assistant (PTA) if required.
  6. Collaborate with physicians, managers, and staff across the organization.
  7. Work in a fast-paced outpatient orthopedic setting with a flexible schedule, including variable shifts and multiple locations.
  8. Maintain a safe environment for staff and patients.
  9. Participate in continuing education to maintain licensure and share knowledge with staff and the community.
  10. Perform other duties as assigned.
Education and Experience:
  • Graduate from an accredited Physical Therapy program; Bachelor's Degree required, Master's or Doctorate preferred.
  • Active Minnesota state license in Physical Therapy.
