Job description

The Physical Therapist is responsible for delivering exceptional patient care by performing evaluations and developing treatment plans for a variety of outpatient conditions.


This is a full-time position with flexible hours at Anytime Fitness in Maple Grove.


Viverant Physical Therapy, part of Infinite Health Collaborative (i-Health), aims to give patients more control over their healthcare through a network of local medical practices owned and led by physicians.


Essential Functions:
  • Evaluate and treat outpatient conditions in consultation with physicians
  • Occasionally evaluate and treat under direct access
  • Use therapeutic exercises, modalities, manual therapy, and other treatments in accordance with state laws
  • Ensure lawful and ethical use of therapy services
  • Document patient care according to guidelines
  • Supervise a Physical Therapist Assistant (if required)
  • Collaborate with healthcare staff across the organization
  • Work effectively in a fast-paced outpatient orthopedic setting
  • Maintain a flexible schedule based on clinic needs
  • Ensure a safe environment for all
  • Participate in continuing education to maintain licensure
  • Share knowledge gained from education with staff and community
  • Perform other duties as assigned

Education and Experience Requirements:
  • Graduate from an accredited Physical Therapy program; Bachelor's degree required, Master's or Doctorate preferred
  • Active Minnesota state license in Physical Therapy
