Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients across ages, modifying plans of care as needed. The role offers a sign-on bonus and relocation assistance for qualifying candidates.

The candidate will provide education to patients and families, develop and adjust care plans, and document outcomes in the therapy EMR. Relocation support is available for moves over 50 miles, and new grads or those with experience are encouraged to apply.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance from bed, chair, and floor; uses safe body mechanics.
• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Completes objective testing for ROM, strength, and daily tasks.
• Documents in therapy EMR and communicates progress with referring providers.
