Qualifications

• Licensed Physical Therapist (PT) - IL
• Basic Life Support (BLS) within 30 days
• Continuing education in acute stroke care

Responsibilities

• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects appropriate examination methods.
• Produces documentation per CTSCP67 policies.
• Screens patients to determine therapy needs and effectiveness.
• Uses support personnel per standards and provides feedback.
