Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, with emphasis on orthopedic and neurologic conditions. You will develop and adjust plans of care, document outcomes, and collaborate with a multidisciplinary team in a hospital setting.

The role requires active PT licensure and BLS, with a sign-on bonus and relocation assistance offered for eligible candidates moving 50+ miles.

Qualifications

• Licensed PT required by state licensing board.
• BLS certification within 30 days of hire.
• Completion of required continuing education for acute stroke care.

Responsibilities

• Evaluates patients with orthopedic and neurologic conditions to establish a plan of care.
• Designs treatment plans integrating goals, outcomes, and discharge planning.
• Performs examinations and selects appropriate assessment methods.
• Educates patients and families using effective teaching methods.
• Documents care and outcomes in the EMR per policy.
• Coordinates with referring providers on patient progress and recommendations.
• Monitors care delivered by support staff and provides feedback.
