Job summary

Advocate Health seeks a licensed Physical Therapist to deliver patient care and treatment within the hospital setting. The incumbent evaluates patients, develops treatment plans, and collaborates with the care team to meet goals.

Responsibilities include documentation, adherence to standards, and active participation in process improvement while maintaining productivity and quality care. CPR/BLS certification is required and relocation support is offered for eligible candidates.

Qualifications

• Current licensure in the state of practice.
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to hospital standards and promotes teamwork through communication and policy adherence.
• Provides patient care assessment and treatment with a sound, evidence-based plan.
• Maintains accurate documentation including evaluations, progress notes, and discharge summaries.
• Ensures legible documentation and timely updates in patient records.
• Meets department productivity standards and adjusts schedules to patient and department needs.
• Participates in process management to improve quality and efficiency of services.
• Promotes professional development and shares expertise with other healthcare workers and students.
• Communicates effectively with patients, physicians, staff and external customers to ensure understanding.

Education

B.S., M.S., or D.P.T degree from accredited physical therapy school
